Understand your mental health & pain — in about 3 minutes.
Understanding your mental health and pain shouldn't be hard. In two short, private check-ins you get a clearer picture of how you're really doing — on any phone or computer.
Looks at depression, anxiety, bipolar, PTSD & pain (the DVPRS) — together
Built on the M3 Checklist, a validated, gold-standard tool
Works with Medicare, self-pay, or a clinic / organization code
Most screenings look only for depression. The M3 Checklist — a gold-standard tool — evaluates depression, anxiety, bipolar disorder, PTSD, and family stress together.
What we do
How M3 works
Step 1
Assess
Take the short, private M3 review of your mood, anxiety, sleep, energy — and pain, on the DVPRS pain scale. About 3 minutes, on any device.
Step 2
Review
Get an easy-to-read report that shows depression, anxiety, PTSD, bipolar, and pain together — so more of what matters is seen at once.
Step 3
Plan
Take your report to a clinician and find the next step and support that fits you.

About this tool. The M3 Review is designed for adults age 18 and older. The M3 Review is informational and is not a diagnosis. To find Medicare-approved providers visit medicare.gov/care-compare or call 1-800-MEDICARE.
If you are in crisis or thinking of harming yourself, dial or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room.
M3 Mental Health and Pain Review
Free for Original Medicare members. $11 for everyone else (HSA / FSA / card / promo code).
Your well-being may be challenging, but understanding your mental health and pain shouldn't be. Two short check-ins. A clearer picture of how you're really doing. Your personal report is private, easy to read, free for Original Medicare members, and payable with most Health Savings Accounts (HSA/FSA).
Traditional mental health screenings often focus on identifying a single condition, primarily depression. This narrow focus can lead to the oversight and misdiagnosis of other prevalent disorders such as anxiety, bipolar disorder, and PTSD. Utilizing the M3 Checklist, a gold standard and user-friendly tool, allows for the comprehensive evaluation of various treatable mental health issues, including depression, anxiety, bipolar disorder, family dysfunction, and post-traumatic stress disorder.

Mental Health
Take the M3 Checklist to assess mood, anxiety, and daily function in just a few minutes. It's a quick way to check in on your emotional health and identify potential risks beyond depression and recognize anxiety.
Assess Pain
Complete the DVPRS pain scale to understand how physical discomfort impacts your daily life. Mood and pain are often connected; treating one can often help improve the other.
How to pay
Free with Original Medicare
No cost to Original Medicare members for the assessment or follow-up care.
$11 — Card, HSA or FSA
Pay with Visa, Mastercard, HSA, or FSA. We email an itemized receipt you can submit for HSA/FSA reimbursement.

Default patient notification cadence
How often a new patient under your promo code is automatically notified to retake the assessment. Consumers can self-select up to 4 weeks (one month max); as the clinician you can override to 1 week for any patient who needs closer tracking. You can also override per patient in the Recent assessments table below.
Clinical Decision Support (Six-Month Pathway)
CDS is a premium clinical-interpretation feature (Six-Month Care Plan, Bipolar Safety Gate, gateway / functional-impairment analysis) enabled in two steps: M3MentalHealth.org approves your practice, then you activate it here. To request approval, contact support@m3review.com. Current state for your practice: checking….
PHQ-9 depression screening module
When ON, patients at your practice complete the 9-item PHQ-9 (Pfizer 1999) right after the M3. The PHQ-9 total appears alongside the M3 score on the longitudinal chart, and every PHQ-9 answer is preserved in the question-history table. Useful when your protocols already use the PHQ-9 or when you want a second depression-specific severity measure. PHQ-9 has been locked OFF for your practice by M3MentalHealth.org. Your selection here will not take effect. Contact support@m3review.com to discuss.
Two-factor authentication (2FA)
When enabled, sign-in requires both your email code and a second factor (SMS or authenticator app). Strongly recommended for clinicians who view multiple patients' PHI.
Choose your second factor:
SMS
A 6-digit code is texted to your phone each sign-in. Familiar, works on any phone.
Authenticator app
Google Authenticator, Authy, 1Password. No SMS needed — most phishing-resistant.
Email
A second one-time code emailed to a different address (e.g. personal email).
Mobile phone for SMS codes
Note: The full 2FA enrollment flow (SMS verification, authenticator-app QR code) is wired up to your production backend on AWS. In this demo, your preference is saved but the second-factor challenge is not enforced.
Sign out of all devices
Useful if you signed in on a shared computer or suspect someone else has access. You'll need to log back in with a fresh email code on every device.
Download my patients' data (CSV)
Export every assessment for patients linked to your promo code as a single CSV — one row per visit, with item-level responses, subscale scores, bands, gateway, and DVPRS. Each export requires a 6-digit verification code (sent to your registered email or phone) for HIPAA-aligned access control.
M3 Master Administrator — Sign in

It is not the sign-in for a practice administrator — practice admins sign in through the Clinician sign-in flow and access their practice's controls from the clinician dashboard.
Admin passcode *
Restricted to M3 administrators. Two factors: the passcode, then a
six-digit code emailed to the M3 administrator on file

Passcodes are not shown on this screen — contact support@m3review.com

By default the code is only needed on your first sign-in each day. Check this to skip it for 30 days on a trusted device. Don't check it on a shared computer.
M3 Master Administrator Dashboard
M3 company use. Aggregate metrics across every client on this platform — patients, clinicians, practices, and health systems. Use the sections below to manage contracts with each client, configure platform-wide consumer settings, and review usage. In production this view pulls live from the encrypted AWS-hosted database.
Clinical Decision Support — Rules Corpus
The rules and evidence base that govern how M3 CDS generates guidance. CDS is educational decision-support for clinicians — it never prescribes or diagnoses.
Governance
CDS authority
Locked to M3 Admin. Clinicians and health systems cannot self-enable CDS. Two steps: M3 grants approval to a practice, then the practice activates it. Off by default platform-wide.
Per-clinic override
A per-clinic explicit setting wins over the platform default (order of precedence: per-clinic value → platform global default).
Inputs
CDS inputs
M3 + DVPRS only. PHQ-9 is a complementary depression screen shown on the longitudinal chart but does not influence tier selection or care-plan messaging.
Bipolar screen
M3 items 20–23 (mania / hypomania). Elevation routes toward bipolar-spectrum guidance and away from antidepressant monotherapy.
Safety trigger
M3 item 5 (thoughts of suicide) triggers the 988 / crisis pathway regardless of tier.
Triage — medication tiers
Tier 1 (~65%)
Depression / anxiety predominant (items 1–7, 8–12); bipolar screen negative. First-line SSRIs; where DVPRS is elevated, an SNRI (duloxetine) covers mood and pain.
Bipolar-depression pattern. FDA-approved bipolar-depression agents (cariprazine, lumateperone, lurasidone, quetiapine). Bipolar II depression: lumateperone or quetiapine only.
Tier 4 (~5%)
Serious mental illness — mania, psychosis, or active suicidality (item 5). Lithium ± antipsychotic; urgent specialist referral.
Pathway & monitoring
Six-Month Pathway
Staged care-plan messaging across visits, clinician-side.
Reassessment cadence
Default 4 weeks platform-wide; a medication rule may tighten to ~2 weeks early; each clinic can adjust per patient. Use validated, measurement-based scales (M3 + DVPRS).
Evidence basis
Guidelines
NICE NG222 — Depression in adults: treatment and management (NICE, 2022); CANMAT 2016 MDD Guidelines (Kennedy SH et al., Can J Psychiatry 2016;61:540–560) and 2023 update; CANMAT/ISBD 2018 Bipolar Guidelines (Yatham LN et al., Bipolar Disord 2018;20:97–170); APA Clinical Practice Guideline for the treatment of depression (2019).
Drug indications
FDA-approved indications and labeling for every named agent; matching uses on-label indications and the patient’s own responses only.
Scope
Educational decision-support only — not a prescription or diagnosis. Clinical judgment and full current labeling govern.
Rules & financial configuration for this category.

Global practice schedule
Platform-wide reassessment cadence default. New patients automatically inherit this cadence. Use the bulk-apply button to reset every patient on this device, or the per-patient field to reset a single record by ID. Consumer maximum is still enforced (4 weeks) for self-service — anything tighter or looser becomes a clinician-override on each affected patient.
One back office — clients across both paths
Every client once, whichever site they were entered on, with the mental-health
and dementia switches side by side and one answer on payment qualification.
The clinical records stay apart: the dementia path keeps its own store and no
assessment crosses between the paths.
Clinical Decision Support (CDS)
Controls whether extended clinical interpretation surfaces on the patient report (the "Note from your care team" letter and the clinical gateway/functional-impairment analysis) and on the clinician dashboard. You can toggle CDS for all consumers, for all clinicians, or override individual records below.

Consumer Platform Controls
Central hub for what the consumer (patient) sees on the platform. Changes apply platform-wide; individual practices can override specific items from their own clinician dashboard where noted.
Medications module
When ON, patients answer the medication-list and side-effect questions after the DVPRS and before their results. When OFF, the medications page is skipped entirely and the patient goes straight from DVPRS to results — useful for screening events, anonymous deployments, and clinics that capture meds outside the platform.
Show the medications page in the assessment flow

Clinical Decision Support (Six-Month Pathway)
When enabled, the consumer-facing Six-Month Care Plan appears at the bottom of every patient's results page, and the clinician-facing Six-Month Pathway appears on the detailed longitudinal report. Recommendations are sourced from NICE NG222 (UK, 2022, amended December 2025), the CANMAT 2023 update (Canada), and the APA Psychological 2019 depression guideline. Default: ON.
Retired (v30.6) — CDS is clinician-only and is switched on per clinician account, never for everyone
PHQ-9 depression module — independent consumers only
Optional 9-item Patient Health Questionnaire (Pfizer 1999). This toggle controls PHQ-9 visibility only for independent consumers (self-paying, no clinician attached). Clinicians and health systems enable PHQ-9 for their own patients from their respective dashboards. When enabled for a given patient, the PHQ-9 appears after the M3 and before the DVPRS; scores 0–27 appear on the longitudinal chart.
PHQ-9 for independent consumers —

SDOH Review (AHC HRSN social-needs screening)
Optional 26-item Health-Related Social Needs screening (AHC HRSN Screening Tool, Center for Medicare & Medicaid Innovation). Covers housing, food, transportation, utilities, safety, financial strain, employment, support, substance use, mental health, and disability. When enabled, the SDOH Review appears as an extra step in the assessment, and a separate SDOH report (questions, responses, and flagged needs) appears on the clinician's detailed longitudinal report.
SDOH Review module —

MMSE Cognition Assessment (SMMSE)
Optional Standardized Mini-Mental State Examination — a 30-point clinician-administered cognitive screening covering orientation, registration, attention, recall, language, and visuospatial ability. When enabled, the MMSE scoring interface appears as an extra step in the assessment, and a full-page MMSE report (item scores, totals, interpretation, and M3 context) appears on the clinician's detailed longitudinal report.
MMSE Cognition module —

Care-plan intake questions
Two short questions that surface on the patient's About-you page when enabled. Their answers are saved with each assessment and displayed in the patient's longitudinal report so the clinician sees context alongside the M3 scores.
Anonymous platform analytics
Counts and distributions across every anonymous review completed on the platform. Data is de-identified by design: age range, sex, scores, and source campaign — no name, email, or contact info. Use this to track adoption of QR campaigns and the score profile of the screening population.
Direct-access anonymous QR code
Pre-built QR pointing at https://m3mentalhealth.org/anonymous. When scanned, the visitor lands straight on the anonymous M3 + DVPRS check-in — no welcome page, no sign-up, no payment step. After completion they tap Download PDF to keep their results. The PNG below is embedded in this HTML file, so you can re-download it at any time without a network connection.
QR encodes
https://m3mentalhealth.org/anonymous
Heads-up: The QR scan only works once m3mentalhealth.org/anonymous is actually serving the V7 build (AWS deployment + a CloudFront rewrite from /anonymous to /?event=anonymous). Test by opening the URL on your phone before printing.
Custom event QR codes
For community screenings, charity events, social media campaigns, walk-up kiosks. Patients who scan an anonymous QR are NOT asked for name, email, or Medicare info — only age range + sex. They complete the M3 + DVPRS and see results on screen. Anonymous scores aggregate in this dashboard but are not tied to any clinic.
Event label
Lowercase letters, numbers, dashes. Used in the URL and as a filter in your dashboard.
Platform URL
Where the platform is hosted. Will become <url>/?event=<label>.
Include the SDOH review — the scan-in flow runs the M3 and then the 26-item SDOH screen.
Test data tools
For demos and onboarding. Seeding creates 5 sample patients (3 individuals + 2 linked to Dr. Sarah Chen) so every dashboard has data to display.
Contracts & Billing
Business relationships with practices and health systems. Each contract is a pool of assessments at a negotiated rate, expiring 2 years from purchase. One assessment = one completed patient session. M3 + DVPRS always issue together and count as one; the optional PHQ-9 add-on is still one assessment. Anonymous assessments do not draw from any contract pool.

There are options.
This is a brief screening, not a diagnosis. Two short questionnaires can give a snapshot — they can't replace a conversation with a healthcare provider who knows you.
Whatever your scores, there are good options. The next time you see a doctor, you could bring this up. Therapy and medication both help most people. Many concerns ease with the right support.
The details above are informational only — not a diagnosis and not intended for self-care.

Six questions. A positive screen is not a diagnosis.
How much has each experience distressed or bothered you during the past month?
Eighteen questions — 0 = Not at all, 1 = A little, 2 = Moderately, 3 = A lot, 4 = Extremely. A positive screen is not a diagnosis.
